Consent to Treatment

I hereby consent to receive medical treatment from Tendon Recovery Clinic, including but not limited to assessment, physical therapy, rehabilitation exercises, manual therapy, osteopathic treatments, acupuncture, taping, shockwave therapy, and any other appropriate interventions deemed necessary and agreed with the healthcare provider and staff members under the employ of Tendon Recovery Clinic, including associates and other healthcare providers.

 

Confidentiality

All patient information and records will be kept confidential in accordance with the Data Protection Act 2018. Information may only be disclosed with your consent or as required by law.

 

Treatment Risks

I acknowledge that no guarantees have been made regarding the outcome of the treatment. I understand the potential risks and benefits of the proposed treatment methods mentioned above and have had the opportunity to ask questions.

 

Financial Responsibility

I am responsible for all charges associated with my treatment at Tendon Recovery Clinic. Payment is due at the time services are rendered unless alternative arrangements have been made in advance.

 

Cancellation and Rescheduling

I understand that I must provide at least 24 hours' notice if I need to cancel or reschedule an appointment. Failure to do so may result in a cancellation fee for the full amount of the appointment.

 

Compliance with Clinic Policies

I agree to comply with all clinic policies and procedures, including but not limited to safety guidelines, code of conduct, and appointment scheduling policies, as enforced by staff members under the employ of Tendon Recovery Clinic, including associates and other healthcare providers.

Governing Law

This agreement shall be governed by and construed in accordance with the laws of the United Kingdom. Any disputes arising out of or in connection with this agreement shall be subject to the exclusive jurisdiction of the UK courts.

 

Changes to Terms and Conditions

Tendon Recovery Clinic reserves the right to modify these terms and conditions at any time. Patients will be notified of any changes.
